To be considered for admission to the Phoenix House you must have a state substance abuse ASAM assessment. You can get an assessment done at your local Mental Health Center or if you are in the Tuscaloosa area you can call Phoenix House for an appointment at no cost to you. If you are in the Tuscaloosa area and you have a family member or friend in jail who needs an assessment you may also call Phoenix House and we will send out a qualified assessment specialist to the jail.

If you already have an assessment fax it to Phoenix House at 205-758-3803 attention INTAKE or email to intake@phtcl.org for review. If you meet the criteria and are appropriate for one of our levels of care our admission coordinator will assist you with what you need for admission to our program as soon as we have an available bed.
